Whether you are coming in for an emergency visit or a regular exam and cleaning, we offer our patients a gentle, personal touch.  On your first visit, you can expect the following:

• Completion of New Patient forms (see below) 

•  A review of your health history 

•  A blood pressure screen 

•  Digital X-rays 

•  A comprehensive exam by Dr. Yarbro

After completion of the above, Dr. Yarbro will determine if any further treatment is needed.

To expedite the check-in process, your new patient forms are listed below.  In addition, if you are on prescription medications, it is helpful to bring in a list of what you are taking, including dose & times taken.  Feel free to print the following forms out and bring them along to your first appointment:

• Patient Information & Health History Form

• In Case of Emergency,Insurance Information, and Consent for Treatment Form 

• Financial Policy Form

• Patient Cancellation Form

• Disclosure of Information Policy Form (only last page is needed)
